Office Policies

We recognize that everyone’s time is valuable. For that reason, we see patients by appointment only. To schedule an appointment, please call our office as far in advance as possible, stop by the reception desk following your office visit, or visit Appointment Request on this Web site. If you are unable to keep an appointment, please give us at least 24 hours’ notice. This courtesy makes it possible to release your scheduled time to another patient.

We try to maintain the scheduled appointment times, but urgent situations sometimes disrupt the schedule. We ask for your understanding and patience during these delays. We will make every effort to keep your waiting time to a minimum.

If you have a large number of concerns or a complex problem, let the receptionist know. If we have advance notice of your needs, we can set aside an appropriate amount of time for you.

URGENT PROBLEMS
If you have an urgent problem at night or during the weekend, call (412) 942-8500 to reach MLIM’s answering service. The service, which answers at all hours when the office is closed, will reach the physician on call.

PERSONAL HEALTH ADVISOR
If you are a Keystone, SelectBlue, or SecurityBlue patient, you have a personal health advisor available to you at all hours. Telephone your advisor directly by calling the toll-free number 888-258-3428.

To call us on a nonemergency basis, telephone Monday through Friday from 8 a.m. to 4:30 p.m. MLIM staff is trained to answer many of the questions you may have. For a list of contact names, visit Feedback to MLIM, a page in this Web site. If necessary, staff members discuss your concerns with a physician. You will receive a response by telephone as soon as possible.

Refill requests will only be processed electronically. If you are in need of a medication refill, you need to contact your pharmacy who will, in turn, send us an electronic request to refill your prescription. Please monitor your remaining medications to allow for at least a 48 hour turnaround time. Please do not wait until you are completely out of a medication to request a refill.

To keep our fees and expenses down, payment for an office visit is due the day of your appointment. A service charge applies to all accounts that are 28 days past due. We urge you to discuss any financial problems with a billing specialist. If you do not have health insurance or are having financial difficulties, we can arrange a payment schedule that suits your circumstances.

MLIM charges a minimal fee for some telephone consultations and for completion of disability, workers’ compensation, and other forms.

Bring your insurance card to every office visit. When you sign the insurance form at our office, you allow us to release information to your insurance carrier; this information ensures that we receive proper payment. Insurance coverage varies from company to company, and you will be billed for any amount not covered. Our Billing Department will file claims with your insurance carrier, but you are ultimately responsible for your bill. If your insurance company does not pay the claim within 60 days, you are responsible for payment of the full balance.

Medicare.
MLIM physicians participate in Medicare. We will complete and submit claims directly to Medicare for services provided at MLIM. You, the patient, are responsible for your annual deductible and the 20 percent coinsurance payment. If you have secondary insurance to cover the coinsurance payment, MLIM can submit the relevant claim forms for you.

Highmark Blue Cross Blue Shield.
MLIM physicians participate in Highmark Blue Cross Blue Shield and accept the insurance allowance for covered services as payment in full. You are responsible for any applicable deductible, coinsurance payment, payment for services not covered under your particular plan, and payment due as a result of income limit requirements.
